ADO forms updated!

As discussed in two of our last PCC meetings, the current ADO forms were hard to keep in stock, hard to read, and often times were not getting to all of the right people. This process has been streamlined and the correct ADO form now lives on Web Form Imprint! (Forms Fast at the hospital). Now, you can print one out, fill it in, and turn it in to your manager. They are then directed to email a copy to the correct people, to include your union officers. We would advise you to fill it in, make a copy for yourself and also submit a copy either electronically or in paper form to Kate Marmorato(co-president) just to make sure we are in the loop. Also important: If you submit an ADO plan to attend the next PCC meeting to discuss your issue. If you are not there, it will be a very one sided discussion and a resolution might not be found.
